Medical Records and Reports - sending them to us:

1. We often request medical reports and medical records in order to assess an applicant’s fitness for an aviation medical certificate. These reports and records may also be forwarded to the CAA or other Authority Medical Department.

Reports and records can be posted to us, or scanned and e-mailed; in all cases they must be written in English, or professionally translated by an accredited translation agency into English.

2. By post: if you opt to send by post, please send to Forward House, Rooms 18,19 & 22, Birmingham Airport, Cargo Terminal, Birmingham B26 3QT. Applicants seen at Harpenden may also post their medical records and reports to Birmingham.

NB: sCANNED AND EMAILED RECORDS / REPORTS MUST COMPLY WITH aLL OF THE FOLLOWING REQUIREMENTS.

IF YOU CANNOT COMPLY WITH THESE REQUIREMENTS, PLEASE POST YOUR REPORTS.

i) The scanned documents MUST be in PDF.

ii) Send ONE document per file i.e. a report from 2 different doctors should not be scanned combined in one file, but should be sent as 2 separate PDF files.

iii) Send multi-page documents as a single PDF file.

iv) Document title format: Surname-CAA Ref No Name of Author-Type of Report-Date of the Report (without punctuation, eg Smith 123456A Dr Jones Cardiology report 26JUN20). Some documents may contain 2 dates, eg clinic date and date letter was typed – use the date the letter was typed. For 24 hour ECG or BP recordings use the start date.

v) The scanned documents must be of good quality. The document text, the applicant’s details (name, DOB etc) should be clearly visible. The report author’s details should also be clearly visible.

vi) The orientation should be correct.

vii) Scaling should be 100% i.e. if document is printed out, it is about the same size as the original, without extra borders.

ix) Attach the pdf files to an e-mail addressed to admin@aeromedicalconsulting.co.uk – do not include the files within the body text of an e-mail.
