Medical certificates are issued on the same day.

AND /OR Complete an EASA application form(if applying for an EASA cabin crew medical)
Complete a Mental Health Questionnaire (do not date or sign)

Your completed EASA application form (if applying for an EASA medical)
Your completed mental health questionnaire (do not date or sign)
Your glasses or contact lenses, if you wear them
Your medical reports relating to any significant current or past medical history
