We carry out revalidation or renewal UK class 3 (ATCO) medicals for existing medical certificate holders at at either our Birmingham Airport or Harpenden (near Luton Airport) clinics.

Photo ID
A UK CAA Cellma online application form (if applying for a UK CAA medical. You do not need to bring a printed copy)
A completed mental health questionnaire (do not date or sign)
Glasses or contact lenses, if you wear them
Medical reports relating to any significant current or past medical history

Bring a completed eye exam form from an optometrist if not submitted in the past 2 years. Use:
