A UK CAA Cellma online application form (if applying for a UK CAA medical. You do not need to bring a printed copy)
A completed mental health questionnaire (do not date or sign)
Your glasses or contact lenses, and your most recent eye prescription (if applicable)
Medical records relating to any significant medical history.

Eye drops may be used that cause temporary glare or blurred vision. If you drive, only drive once your vision has completely returned to normal.
Applicants with previous LASIK or refractive surgery must provide a report to include dates, type, refraction, and complications.

We are one of only three Aeromedical Centres in the UK approved to certify applicants for an initial UK & EASA commercial pilot’s medical certificate, and are the only AeMC outside London and the South East with this approval.
